The Karian Dam Project is one of a total 48 dam construction strategic projects in Indonesia that have experienced delays. Based on Addendum 6, the targeted for completion March 2021. However, delays several works. This study discusses factors delay, location risk and work item assistance causes Project. research method used this quantitativ. Data collection was carried out using questionnaires interviews. obtained data then analyzed Fault Tree Analysis (FTA) to find source cause delay. subsequent analysis uses Failure Mode Effect (FMEA) assess risks been identified FTA capture. According findings, there are 47 potential sources delay method. After calculating probability index, it found activity A9-1 (planning implementation) has highest index value 4. FMEA obtains Risk Priority Number (RPN) R-42 change) with an RPN 64, arrest search matrix, as many 9 at high (high risk).
